Form to be the input for creation Jobs

Hi there.
First of all, i apologise for my bad english, i will try to be simple and clear.

I need to have an Document or any kind of Form to share to our costumers, for them to fill in order to us to create Jobs in Control-M.
Does anibody have this?

hello ,,

you could use the below as different columns in a excel sheet and ask the client / customer to fill in the data for you.
